1.  Introduction

This module will be assessed based on the core module learning outcomes:

LO 1: Examine the relationship between the ageing population and the health care priorities within nursing practice today.

LO 2: Evaluate evidence-base for the assessment, planning and evaluation of holistic and compassionate care for older adults and their families.

LO 3: Evaluate the concept of healthy ageing and the national and international strategies for health promotion.

LO 4: Critically explore the effectiveness of nursing theories and approaches to working with older people who have complex needs.

The format of the assessment is an essay with the inclusion of a care plan related to a relevant case study. The assessment is designed to allow you to meet the four learning outcomes.

2. Details of Formative Assessment

Formative assessment: an opportunity for formative feedback is offered via the submission of a case study. This will offer both formative feedback opportunity from academic staff and peer group feedback to support learning and support the development of the summative assessment (This links to Learning Outcomes 1 & 2).

Formative work is not marked and does not contribute to the final mark for the module. However, it will provide you with essential feedback on a number of aspects of your academic skills and writing in line with the module assessment. These include:

By submitting your formative assignment you will ensure you are on the right lines in relation to your preparation for your final submission as well as reassurance that you are writing and referencing in line with the expectations of degree level work.

The case study you include in this formative submission will be included and used in your final essay. The template provided must be used to complete the formative assessment.

2.1. Selecting a Case Study

For this assignment, you should be able to identify someone you are looking after or have looked after in the past.  It is much better if you can relate your assignment to your current clinical practice.

Confidentiality needs to be maintained throughout your work, so it is important to clearly state in your case study description that the name of the individual has been changed in line with confidentiality guidance.

The case study is your choice. There are no restrictions other than it must be an older person. This would mean someone over the age of 65 years.

Anyone who is unable to identify a case study from their current or previous practice experience should contact the module leader and discuss this with them. We have included some case studies here  but they should only be used if you really don’t have a patient you can use as an example. Please note that you should not copy these case studies word for word, but should use them as a basis for getting started.

The case study does not need to be overly complex in relation to health needs, as long as you can relate to it and address the requirements for the assignment. However, the patient does need to have some health conditions which you can include in your care plan.

Assignment tasks are embedded in Moodle learning units to help you identify and decide on a relevant case study.

3. Summative Assessment Details

The Summative Assessment will be in the form of an essay that will examine the theoretical and legislative perspectives of positive approaches to nursing care, that support older people making use of the agreed case study to demonstrate the application of this knowledge. (2,500 words – LO 1,2 & 4). This assignment will also include a 1,000 word plan of care relating to the case study submitted as the formative element (LO 3).

The Summative Assessment is the final, marked assessment that contributes to 100% of your mark for this module. A Summative

Assessment is required to be aligned to your module learning outcomes and marked in line with the success criteria which is available in this assessment handbook.

There are two main sections to this summative assessment and these are:

Essay: The essay is based on the case example you have used in your Formative Assessment and will be the base on which you demonstrate your understanding and application of theory to inform and guide the practice included in the module: Ageing theory and nursing theory. The theories selected should promote positive approaches to working with older people.

The second part of this essay will be the completion of a care plan that demonstrates your awareness of applicable health assessment including the dimensions of health, planning of care and suggested interventions that will improve health outcomes.

Here you will find a care plan template . You must use this template for the care plan when you write your summative assessment.  It is a

Word document, and will expand as you add content to it.  Please note that there is no template for the essay.

Promoting Excellence in Older People’s Care – Assessment Specific Guidance:

You will have two assessments. A Formative Assessment and a Summative Assessment.

Formative Assessment

The Formative Assessment is a piece of word that is not given a mark. The purpose of a Formative Assessment is to provide essential feedback which will inform the development of your final marked summative submission and help you to develop academically.  Feedback will be in the form of general and specific feedback on academic writing, focus, referencing style and overall understanding of the module requirements.

The formative assessment for this module is the completion of a short essay outline and a template (available in Moodle) is provided which identifies the sections.

Summative Assessment

The Summative Assessment is the final, marked assessment that contributes to 100% of your mark for this module. A summative assignment is required to be aligned to your module learning outcomes and marked in line with the success criteria which is available in this assessment handbook.

This will be in the form of an essay that will examine the theoretical and legislative perspective of positive approaches to nursing care that support older people.

Your essay will focus on the agreed case study (outlined in your Formative Submission) and you will discuss international and national policies that relate to older people’s health, nursing and ageing theories that relate to older people’s healthcare and the senses framework. It is important that you apply this knowledge to your case. For example, you may choose to discuss person-centred care. You would explain what this is and support your statements with relevant literature about the topic but also relate this to your particular case study.

The assignment will include a care plan relating to the case study. A template is provided to help you outline some key priorities of care (up to 5). You are asked for a rationale for care decisions and here we would like you to mention relevant literature that informs this. For example, one presenting problem and priority for care might be to manage their pain effectively. The template asks for suggested nursing interventions and your short-term and long-term goals but also a rationale for your decisions of care. This would include a reference that supports the pain management strategy that you suggest.

The work submitted will be 3,500 words (+/- 10%) in total. 2500 words for the essay and 1000 words for the care plan.

Structure for the Assignment

Section 1 – The essay

This should be written in the third person and should be based on the case example you selected and presented in your formative assignment.

The aim of the essay is to allow you to demonstrate your understanding of the unique needs of older people and how nursing and ageing theory can help inform and influence practice. We suggest that you write a short overview of what nursing and ageing theory is (you should not try to explain each of the theories but may give examples) and then explain how this may inform care in the case of the patient that you have chosen to discuss.

You should also refer to policies that direct and promote excellent care for older people and enhancement of their health and wellbeing.  Give examples of how one or some of these policies apply to your case. We are looking for you to demonstrate some in-depth thinking and reflection so do not try to cover too many.

Section 2 – The care plan

Provide a short overview, which could include the purpose of the care plan. This should be referenced with appropriate literature.

Complete a clear plan of care using the template and supporting references.

Referencing

Following your care plan you should complete a short conclusion to the essay and then include the reference list in APA 7th style.

Submission

All work both formative and summative should be submitted via Turnitin in Moodle on or before the due date/time.

There will be a “TEST” Turnitin box open for you to use to check your work for similarity – please be aware that this box will close 24 hours before your final submission deadline.

Introduction

 

Write an introduction to your essay. 5 marks

 

The introduction should act as a guide to the reader and help signpost them through your essay. This section should outline precisely the areas you will be talking about. For example, if you simply talk about nursing or ageing theories, this is not sufficient, you need to identify and describe how this theory relates to the care you provide and or the assessment of the people you are nursing.

This section should be no more than 200 words.

 

Case study

 

Outline briefly your case study that you will use to illustrate the theories and care plan for this essay. 7 marks

 

The case study should outline a person and their presenting problems some of which you will discuss in the essay and the care plan. It needs to be detailed enough so that the reader can identify the issues you discuss in relation to the theories and care plan.

 

Presentation in the essay: You can do two things here:

Outline the case in two or three sentence e.g. the case-study is of a 94 year old woman, Mrs. Tau who has fallen and has been admitted to hospital (see appendix 1).

 Alternatively, you can put this in a paragraph in the essay using no more than 200-300 words.

 

Main body

Is divided into four sections

Section 1:

This should discuss international and national policies that relate to older people’s health and your case study.

15marks For this section you should look at Global policies such as WHO in terms of older people’s health.  You then need to discuss and evaluate national and local policies relevant to your country on older people’s health health and your case study. You then need to broaden the discussion and relate the national/local policies you have chosen to your case study.

 

For example: You might want to take one or two examples and critically discuss them exploring the impact on your case study and how this aligns with older people’s health generally.

 

This section is approximately 600 words, written in academic style and must be supported by evidence from your reading.

Section 2:

You will provide an overview of the theories and discuss one ageing theories that relates best to your case.  Explain how it fits with your case study

15 marks In this section there is an expectation that you demonstrate that you are aware of the various theories of ageing.

You will then consider discussing one ageing theory that best applies to your case. There is an expectation that the theory you choose would be critically discussed – so the strengths and weakness of the theory. There is a requirement to link discussions of the theory to the case study.  So you need to explain how the theory fits with your case study.

   

This section is approximately 600 words, written in academic style (3rd person) and supported by evidence from the literature.

Section 3:

You will provide an overview of the theories and discuss one nursing theory that relates best to your case.  Explain how it fits with your case study

15 marks In this section you may want to make reference to examples of key nursing theories that are available relating to older people’s health care. You will then choose one theory to discuss in detail and relate to your case.

For example, you may choose to discuss person-centered care. You would explain what this is and support your statements with relevant literature that focuses on the use of person centered care. Make sure you apply this to your particular case.

This section is approximately 600 words, written in academic style (3rd person) and supported by evidence from your reading. 

Case study You need to develop a care plan relating to the case study. 26 marks Here you are expected to develop a care plan looking at no more than 5 key priorities.  However, to get some depth it may be better to discuss 3-4 key priorities. A template has been provided already to help you outline some key priorities of care.

 

You will see that the template asks for suggested nursing goals, interventions but also a rationale for your decisions of care.  This would include a reference that supports the intervention you suggest.

 

For example, one presenting problem/ nursing need and priority for care might be to manage their pain effectively. You may choose a pain management strategy and here you would need to evidence base this with a reference from the literature.

References should be provided in accordance to the School of Health and Social Care (SHSC) guidance.
Structure Evidence of a clear structure and sequence in the essay presentation. 7 marks Marks will be allocated for the structure and sequence of the work; in other words the material is presented in a logical manner and is linked together.
