Russell is a 45-year-old property agent who has been experiencing persistent\ low mood and a lack of interest in daily activities for the past five years. He reports feeling down, fatigued and hopeless for most of this time, with no extended periods of relief. Russell’s symptoms have significantly impacted his work and social life, and he finds it challenging to maintain relationships or concentrate on tasks.

He can recall a couple of instances where his mood dipped significantly but he reports that he has always felt “down”. He asks for your expert opinion on whether his experiences are a result of job stress, if he is experiencing a mid-life crisis, or if it is a psychological diagnosis which he has. He awaits your assistance.

(a) As his counsellor, help Russell better understand the symptoms which he is experiencing. Appraise TWO (2) plausible DSM-5 diagnoses which he could have. For each diagnosis, highlight TWO (2) relevant symptoms from the case study, and provide TWO (2) other plausible symptoms from your own readings about the respective diagnoses. In addition, organise the information which Russell has provided to answer his questions on whether he is solely experiencing job stress, or a mid-life crisis.

(b) Using Cognitive-Behavioural Therapy, develop an intervention plan to best  help Russell alleviate the concerns and symptoms he is experiencing. Your intervention plan should be realistic given your resources as a counsellor. Provide specific examples of what will be carried out. The intervention plan should address as many of Russell’s presenting concerns as possible, and have a well-defined structure and timeline.
