Assignment Brief:
Each student is to research and submit a written assignment report of the following accident involving aircraft VH-NGA and the circumstances associated with the collision with terrain accident reported by the Australian Transport Safety Bureau (ATSB) in its Aviation Occurrence Investigation, AO-2009 (reopened) Final dated 23 November 2017.
Following the format in Table 1, the report is to provide your analysis of what occurred and discuss salient gaps and deficiencies from a human factors perspective using Reason’s Swiss Cheese Model and any other 1 of the other following prescribed human factor models (SHELL model, SCHELL model, PEAR model, Dirty Dozen model, HFACS model).
