Question 1

Case Scenario

The Straits Tines on 27 March 2024 reported that 36 workers died and 590 major injuries occurred in 2023. The construction sector saw the greatest number of deaths in 2023. Injuries were mainly due to machinery incidents, or slips, trips and falls. MOM reported that common causes included companies’ lack of control measures and workers not complying with safety measures. Falls from height were one of the main reasons for accidents, causing 8 deaths and 67 cases of major injuries at workplaces. The rate of occupational diseases rose from 29.7 per 100,000 workers in 2022 to 33.6 in 2023.

With this information discuss the following: 

a) Examine and describe the role and responsibilities of the Employer as covered by the Workplace Safety and Health Act.

b) Describe the various methods which must be put in place to prevent workers from falling from heights.

c) Examine and describe the different types of occupational diseases that can occur at workplaces and discuss the workplace safety which can be instituted to reduce and  prevent occupational diseases.

Question 2

a) Case Scenario 

An incident occurred at a food manufacturing plant when a worker’s hand got caught in one of the food processing equipment. The trip switch of the equipment immediately stopped the processes, and this enabled the other workers to assist with getting the worker trap hand out. A trained first aider quickly provided the initial first aid and the worker was subsequently whisked by an ambulance to the hospital. The injured worker had to go through an urgent surgery and was hospitalised for 3 weeks and given 2 months of medical leave.

Based on your understanding of the Workplace Safety and Health Act and continued safety operations at the manufacturing plant, describe the employer duties, and follow up actions from this accident.

a) The Workplace Safety and Health Design for Safety (DfS) aims to identify and reduce safety and health risks at the planning stages of a project. Appraise and describe the TWELVE (12) important responsibilities of a Developer towards DfS.

Case Scenario

You are appointed as project manager at a construction site and is very concerned with site safety after several near mishaps. As a certified safety officer, you decided to carry out a risk assessment of the construction site. A risk assessment team comprising of a site supervisor, a contractor site foreman, an experience construction worker, and contractor safety officer is formed. You instructed your risk assessment team to submit the risk assessment within 4 weeks and must include the following:

a) Identify and elaborate the work activity and location for understanding on at least TWO (2) hazards which potentially can cause serious accidents.

b) Examine the potential ill health/accident which the hazards can cause.

(4 marks)

c) Recommend and prepare risk control measures which can be implemented to reduce the hazards.

(4 marks)

d) Appraise another risk control measure which can eliminate the hazards.

(4 marks)

e) Prepare a complete Risk Assessment Control Form report to be submitted to the project manager for his approval for implementation of the risk controls recommended by the team.
