(a) Provide a brief background (up to 70 words) of an individual with a psychiatric condition. What is the psychiatric condition? Describe the symptoms experienced by this individual and how the diagnosis and symptoms of the psychiatric condition affect the life of the individual and his/her family. (The individual could be an acquaintance, a friend, a relative, or a person who has written about his/her psychiatric condition and had published it in a journal, newspaper, magazine etc. Please do not interview any person who has a psychiatric condition for this assignment).
(b) Apply one (1) relevant concept learnt in the course to discuss the lived experience and two (2) issues that the individual faces, arising from his / her psychiatric condition.
(c) Based on the two issues discussed in (b), describe two (2) current psychiatric rehabilitation programs in Singapore and analyse how these programs can support the individual cope with these issues.
(d) If you are a Social Worker running one of the psychiatric rehabilitation programs identified in (c), provide a detailed discussion on how you will apply the Wellness Recovery Action Plan to help the individual to prevent relapse and enhance their wellbeing.
