Choose one (1) real life case scenario either from your professional practice or within your personal circle who requires a conversation of an advance Care planning discussion and management. Use the provided headings below to assist with your discussion Introduce your Case Scenario.

A brief overview of your case scenario (Include demographic and background history)

• Briefly discuss their disease process and justification of the need for ACP (you are not required to discuss the Anatomy & Physiology of their disease, rather the focus of this is to explore the role of advance care planning in the context of the chronic illness progression). Identify current diagnosis and how it applies to the progression of chronic illness and why this is the optimal time to discuss the need for ACP.
• Discuss the possible benefits of the ACP pertaining to your case scenario specific to both the patient and family/others.
• Discuss the approach you would take to commence the conversation of ACP, keeping in mind local customs and traditions.
