Case Study Question:

You are seeing Mark, a 43-year-old Singaporean-Indian man, and a recent divorce, for the first time. In your first session together, Mark reports that he is not performing well at work and has been feeling “low”. These “low” feelings surfaced when his immediate supervisor gave him a piece of job-related negative feedback about two months ago. Since then, he has felt a lot of resentment towards his supervisor and feels that the work that he does is worthless. He has trouble falling asleep at night and often wakes in the middle of the night, unable to go back to sleep. He tells you that he is exhausted all the time, has trouble thinking clearly, and has no appetite during most meals. He has lost interest in his usual hobbies and does not feel like leaving his house. He has not showered in two weeks.

At work, he gets anxious easily and has lost confidence in his own performance. At times, he experiences breathlessness, tightness in his chest, and an impending sense of doom. He feels an almost constant sense of hopelessness and tells you he is ready for “things to be over” since everything feels terrible to him. He has stopped talking to his children and has left them to the care of his ex-wife. Next week, he plans to meet his lawyer to discuss his will.

  • Identify THREE counseling issues that Mark might be experiencing based on the information given in the case study.
  • Discuss TWO counseling goals and TWO counseling interventions. You should demonstrate how the goals and interventions relate to the counseling issues identified.
  • Ascertain if it is appropriate to perform a suicide risk assessment, and discuss
    your rationale. Discuss if it is appropriate to refer Mark for medical/psychiatric
    evaluation and follow-up.
