Case-Based Scenario on Health Promotion and Communication

You are a Health Coach at a Senior Activity Centre (SAC). Mr Ahmad, a 68-year-old man, retired cleaner, currently unemployed, has been referred to your Centre for health coaching. He is severely obese and has several health and social issues.

Medically, he had a mild stroke and walks with an umbrella. He has been diagnosed with diabetes, hypertension and a mild stroke. He has stopped taking his diabetes and hypertension medications. He is also on a blood thinner due to his stroke.

Socially, he is single and lives in a one-room rental flat. He depends on public assistance funds. He has no other relatives and is socially isolated. He does not talk to anyone including his neighbours. He keeps to himself most of the time.

Task

Using the “Programme Planning” Model, you are tasked to manage the biopsychosocial issues of Mr Ahmad’s. Your programme plan should include, but not be limited to, the following areas of focus.
