Question 1

In this assignment, students are required to talk to one, or a few medical social work practitioners in healthcare. The settings could be in hospitals (acute or community) or Intermediate and Long Term Care Sector (ILTC) such as home-based (home palliative, home nursing, etc), centre-based (day rehabilitation centres, dialysis units, hospice or psychiatric day centre, etc) or institution-based (inpatient hospice, nursing home, etc)

(a) Write a case study arising from the conversation with the interviewee(s). If it is a real case, students should omit personal identifiers. If it is not an actual case, you may formulate a hypothetical one, based on your interviewee’s account and your observation. The details of the case study should include the presenting problems or reason for referral, and an analysis of the variables (for eg, demographics, family, occupational, socio-economic, psychological). Illustrate your case study with a genogram and an ecomap. Include legends. The length of the case study should not be more than a two-sided page (single line, font 12). (20 marks)

(b) Demonstrate your understanding of the identified illness in relation to the case study. Where applicable, students may look at the nature, symptoms, treatment, goal/duration/cost of care, and/or impact of illness.

(c) Examine four (4) core social work skills specific to the case study. The skills identified should primarily alleviate the biopsychosocial impact for the patient and family, as well as at a macro level of practice. (20 marks)

(d) As you review this case study or cases alike with similar nature or encounter, appraise suitable healthcare policy, funding mechanisms, or service delivery with the aim to enhance the responsiveness of the environment in meeting the needs of patient and family. (30 marks)

(e) Overall structure and organisation, coherence in argument, and accuracy in academic conventions and APA referencing (10 marks)
