Mr. Anthony, a 70-year-old retired policeman has been complaining of persistent cough and constant fatigue for the past 1 year. Recently he has not been eating well and lost 10 kg within 3 months.

CT scan revealed he has non-small cell lung carcinoma in right & left lung and it is inoperable. He has been diagnosed with Stage 3 Lung and esophagus cancer.

Anthony divorced his wife 20 years ago and staying alone in his 3-room flat.  He depends on his monthly pension fees to support his living. Anthony was referred to the Palliative care team for further management.

PAST MEDICAL/SURGICAL HISTORY & MEDICATION:

• Tuberculosis
• DM Type II
• Hypertension
• Hyperlipidemia
• Arthritis
• PVD
• Poor vision
• Medication: Metformin 500mg BD; Losartan 50mg OM; Simvastatin 10mg ON; Calcium with Vitamin D 1-tab BD

FAMILY HISTORY:

• Father died of tuberculosis
• Mother aged 100, taken care of by his brother

SOCIAL HISTORY:

• Smoker of 30 years
• Social drinker
• Does not like to exercise
• Retired
• Divorced
• Only have 2 closer neighbors, aged 72
