      Case study

      Mr Ong is a 61-year-old Singaporean man living with the β€œthree highs” β€” Hypertension (high blood pressure), Diabetes Mellitus (high blood sugar), and Hyperlipidaemia (high cholesterol). These conditions place him at increased risk of cardiovascular disease, stroke, and kidney complications, which can significantly impact his long-term health and quality of life.

      Mr Ong has been on long-term medication to manage his blood pressure and cholesterol, yet his readings often remain above target levels. Diagnosed with type 2 diabetes four years ago, he finds it challenging to keep his blood sugar stable due to irregular eating patterns and a lack of regular physical activity.

      He works part-time as a delivery assistant and lives with his wife and adult son. Although his family provides emotional support, his modest household income limits access to regular medical reviews, medication adherence, and healthier dietary options. With a secondary school education, Mr Ong occasionally struggles to understand complex health information and relies on informal advice from peers.

      Mr Ong frequently eats at hawker centres, drinks beer socially on weekends, and continues to smoke occasionally despite medical advice. As he grows older, the cumulative impact of his conditions and lifestyle habits pose increasing risks to his overall well-being and independence.
